WAIVER AND RELEASE FROM LIABILITY – MINOR PARTICIPANT In consideration of being allowed to participate with Lakeside Dance, LLC, and any related events and activities, including, but not limited to, dance and tumbling classes, performances and rehearsals, and intending to be legally bound, the undersigned: 1. Acknowledge and fully understand that each participant may/will be engaging in activities that involve certain inherent risks, including serious injury, disability and death, and social and economic losses which might result not only from their own actions, but inactions or negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time, including but not related to Covid-19 exposure. 2. Assume all the foregoing risks and accept personal responsibility for the damages related to any such injury, permanent disability or death. This release specifically includes any and all claims relating to any Covid-19 related illness arising from or related to participant or participant’s guardian’s involvement with Lakeside Dance, LLC. The parties acknowledge that although Lakeside Dance, LLC shall utilize best practices to mitigate the spread of such illness to any persons involved with the dance studio, that Covid-19 is a highly contagious pandemic disease out of the control of Lakeside Dance, LLC. 3. Release and agree to hold harmless Lakeside Dance, LLC, in addition to its members, officers, and employees, all of whom are hereinafter referred to as "Releasees," from demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or part by negligence of the Releasees or otherwise. THE UNDERSIGNED HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT THEY HAVE GIVEN UP AND WAIVED SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT VOLUNTARILY.

Students of Lakeside Dance, LLC. may have the opportunity to be photographed or videotaped in class or at various dance events. With permission from parents these photos or video may be included for Lakeside Dance, LLC. publications or advertisements, including on our websites and facebook. To ensure privacy, specific names of children will not be included. No financial compensation shall be given for use of any photographs or video. I acknowledge that Lakeside Dance, LLC. may choose not to use my photo or video at this time, but may do so at its own discretion at a later date. Lakeside Dance, LLC. reserves the right to discontinue use of photos or video without notice. I have read this release and I understand its contents. I consent to and authorize the use by Lakeside Dance, LLC., or anyone authorized by Lakeside Dance, LLC, of any and all photographs or video which have been taken of my child/children.

Do you agree to our photo release?

The world today is such a busy place, so many things to do. Class attendance sometimes suffers because of the hustle and bustle of today’s life styles. The one who suffers the most because of poor attendance is the student himself. The technique taught in class cannot be made up. Technique is what makes a good dancer. Learning to do a dance is not the only purpose of taking dance class. Students also learn valuable life skills such as responsibility, accountability, work ethic, commitment, and team work through dance attendance. The dance class is comprised of much more than what is in one dance number. Our dance routines are made up of technique taught throughout the year. If a student is not in class to learn proper technique, then the dance cannot be performed properly.

By missing so many classes, the student is often unprepared for the recital and does not exude confidence. Many times we have seen students quit midway to late in the season to join other activities. Please note, dance is a team effort. When one child quits the entire class suffers. Students often have to scrap everything they have learned all year only to learn all new choreography in a short amount of time. The class as a whole is frustrated and they do not perform as well at the recital due to one child's choice not be committed for the entire season. Not allowing your child to quit midway through the season and maintaining good attendance teaches commitment, dedication, self-confidence, as well as team effort and responsibility.

You also have the option of taking the class but not participating in the recital. If you know ahead of time that January through May is a busy time for your family, this may be in your best interest and you would not waste money on a costume.
